The Morongo Basin Healthcare District is pleased to present education and support programs to improve the health and wellness of our residents.


This Diabetes Management Program is is an award-winning program, recognized by Health Resources and Services Administration (HRSA), Federal Office of Rural Health Policy, as a model diabetic education program. It combines an "all in one" approach to diabetic education through the inclusion of patient education, group support, sustained engagement and follow-up with participants.

It is a lifestyle coaching program for individuals diagnosed with Diabetes Type-2. The 12-week course includes medical and nutritional information, alternating weeks with supportive behavioral health therapy. The 12-week program is followed by three monthly support sessions. The program is offered twice a year in January and September in both Twentynine Palms and Yucca Valley locations.

This program empowers patients to care for their health through disease self-management activities. This has produced productive interactions during office visits, classroom time and follow-up phone calls. Support of participants to promote good chronic illness self-care is done through educational sessions, informational materials, social media, community resources collaboration, referrals, exam room computers, and website banners to improve functional and clinical outcomes.

After three months of the eight-month program, through managing their diet, physical activity and diabetes health, the first cohort (January, 2018) yielded significant improvement in collected data. After three months, the highest A1C for the class went from 12.2 to 10.2 with total class A1C reduction of 8.1.
